Further I do release and hereby agree to hold harmless, Hillsboro School District, Singing Hills Christian Church and its employees and agents from any and every claim arising, or which may be asserted by me or by any member of my family by reason of participating in any activities associated with the listed child activity.
Further, I do authorize the minister or sponsor of this activity or any Singing Hills Christian Church staff member, in the event I cannot be reached by phone, to give consent to a physician and/or hospital for emergency medical or surgical treatment required by the minor child while on this activity. It is understood that I, the parent or legal guardian, will assume any financial responsibility for any expenses that may be incurred for said emergency treatment.

Optional Balloon Ride - Friday August 17th.
On Friday kids will have the opportunity for a free ride in a hot air balloon. (It will be tethered to the ground for a distance of 10-20 ft.). You can choose to have your child participate. If your child will be participating the Pacific Peaks Balloon Company will also require a release.
