Join us for an exciting week filled with engaging Bible lessons, creative crafts, fun games, and more at our annual Vacation Bible School.

This event is a wonderful opportunity for kids to learn, grow, and have a blast.

Ages: 5 years old through those entering 6th grade

VBS is free to attend.

Vacation Bible School (VBS) Liability/Medical Release, Authorization, Consent Information:

Liability:
I, the above signed parent/legal guardian of the above listed child give permission to participate in the Vacation Bible School (VBS) program and activities hosted by Singing Hills Christian Church.

I understand that participation in VBS activities may include games, crafts, recreation, snacks, outdoor activities, and other group events that may involve certain risks of injury or illness. I voluntarily allow my child to participate and assume all reasonable risks associated with these activities.

In the event of a medical emergency, I authorize the VBS staff, volunteers, or representatives of the Church/Organization to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that every reasonable effort will be made to contact me as soon as possible.

I agree to be financially responsible for any medical treatment provided to my child. I release and hold harmless the Church/Organization, its pastors, staff, volunteers, and representatives from liability for injuries, damages, or losses arising from participation in VBS activities, except in cases of gross negligence or intentional misconduct.

I also certify that my child is physically able to participate in normal VBS activities unless otherwise noted.

Authorization & Consent

I have read and understand this medical release and liability waiver and give permission for my child to participate in VBS activities and I have disclosed any of their allergies and medical needs.

Photo & Video Release

I grant permission for Singing Hills Christian Church to photograph and/or record video of my child during Vacation Bible School (VBS) activities. I understand that these photos and videos may be used for church purposes, including, but not limited to, church presentations, newsletters, social media, printed materials, promotional materials, and the church website, but never for private use or monetary gain.

I understand that no names or personal information identifying my child will be used without additional permission.

By signing above, I acknowledge that I will not receive compensation for the use of these images or recordings, and I release the Church/Organization, its staff, and representatives from any claims arising from such use.
